Traditionally, payor organizations have not been concerned with the social determinants of health and how these determinants impact a patient’s health. What we know however, is that SDOH play a significant role in the cost and effectiveness of care which can impact readmission rates and other quality measures. For this assignment use business letter style to create a letter to an insurance company explaining how capturing and addressing the SN Z codes can improve patient care, prevent acute care readmissions, improve health outcomes, and inform health equity strategies that can lead to lower health costs. Your letter should be a minimum of 3 pages excluding a reference page.
More specifically, your letter should address the following:
Write an Introduction about your role and responsibilities as a healthcare professional to provide evidence of your credibility. They need to understand why your opinion matters.
Describe what the SDOH are and how they impact access to care, affordability of care, compliance with treatment regimes, and eventually health outcomes
Explain how adding SN Z codes to claims data can decrease healthcare costs by identifying and addressing the issues identified by the codes. Use literature to support your assertions with a minimum of five sources from the literature; use intext citations.
Close with a description / recommendation of strategies you believe the insurance company can implement using claims collected SN Z code data to achieve health equity.
